- Advanced Life Support (ALS) is a level of care that usually requires the administration of drugs or other invasive procedure to stabilize a patient.   Basic Life Support (BLS) does not require the administration of drugs. Refusals are patients that were evaluated by a paramedic but chose not to be transported to an area hospital.
